What this site collects, what it never asks for, and how texting works. Written to match what the site actually does.

The short version
The forms on this site ask for a first name and a mobile number, sometimes an email, and what you’re interested in. They never ask about your health. Medical history, medications and photos for your chart go through our booking app, Boulevard, which is built for health information.
We don’t sell or share your details. We use them to text you back.
What the forms collect
- Name and mobile number, so we can text you back
- Email, only if you add it
- The treatment you’re curious about, a preferred time, and any dose map you built
- Which page you sent it from and a short referral code if you arrived from a shared link
The booking form adds the time you picked. The gift card form adds the amount and the note for the card. None of them take payment.
What we never ask for here
Diagnoses, medications, allergies, pregnancy, past procedures, or photos of your face. Those belong in your private intake in Boulevard, which you get by text after you book.
Texting
If you tick the texting box, we text you about your consult and appointments. Message and data rates may apply. Reply STOP to opt out at any time, or HELP for help. The box is never ticked for you, and texting is never a condition of booking.
Analytics and cookies
This site sets no cookies of its own. A live version may add Google Analytics and Microsoft Clarity to count visits and see where pages are confusing. They never load on the booking, consult or contact pages, and no advertising pixels run on any page where you might share health details.
Photos and reviews
No client photo is ever shown on this site or anywhere else without written consent, which you can withdraw. On this concept site, every photo is a sample image and every review is a sample.
How long we keep it
Form messages are kept for 12 months so we can follow up, then deleted. Your medical chart is kept as Arizona requires, inside the booking system.
